Pricing:

• First visit  |  $175  | 60-minute session including evaluation and treatment.

• Follow-Up Visit  |  $150  | 60-minute treatment session.

• First visit  |  $250  | 90-minute session including evaluation and treatment.

• Follow-Up Visit  |  $225  | 90-minute treatment session.

Line items below will be listed on your invoice with the appropriate CPT codes according to your customized treatment plan.

Cancellation/No Show Policy: All cancellations need to be made 24 hours prior to your appointment, or you will be responsible to pay for 100% of the session missed. 

Late Arrival Policy: If you arrive late to your appointment, the full rate will be charged and your appointment will end at the scheduled time.

Payment Policy and Methods:  Payment is due at the time of each visit.  Ashley is an out of network provider and does not contract with any insurance companies.  You may be able to submit your invoice for payment through your insurance company under out of network benefits.  You will need to contact your insurance company for more information and instruction.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. 

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes  related costs like medical tests, prescription drugs, equipment, and hospital fees. 
• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 
• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 
• Make sure to save a copy or picture of your Good Faith Estimate. 

These are the standard charges for care.  This is a cash based physical therapy practice. Upon the completion of care each visit, you will receive an invoice stating you paid for the following services:
